Current Position Openings at Rural Health Network of SCNY, Inc.

The Rural Health Network of South Central New York (RHNSCNY) is seeing a full time Administrative Support Assistant. The position will provide excellent customer service for both external and internal customers. This position is the initial point of most public contact via telephone or walk-ins. Additional duties include purchasing office supplies and acting as point of contact for I.T. contractor. Responsibilities may increase or become more specialized depending on performance and skills. Candidate must demonstrate excellence in customer service, good organizational and problem solving skills and ability to support a wide range of agency and program activities.

Population Health Coordinator, Tioga County: The Rural Health Network is seeking a full time Population Health Coordinator to serve Tioga County, NY. The Population Health Coordinator is responsible for working collaboratively with other Population Health Coordinators and Healthlink NY, the lead agency for the Population Health Improvement Project, a regional population health initiative. Responsibilities include implementation of work plan activities in Tioga County, including workplace wellness initiatives, support for community health planning and needs assessments, support for the development of age friendly communities and active participation in community health forums and networks. Qualifications: Bachelor’s Degree in public health, public administration, human services, etc. and three years related experience, or an acceptable combination of education and work experience. This position is eligible for all full time position benefits.

Social Determinants Project Coordinator: The Rural Health Network is seeking a full time Social Determinants Project Coordinator to implement a regional project designed to improve data collection and measurement of the impact of social determinants of health interventions on health outcomes.  This is a collaborative project that includes healthcare, community, regional and NYS partners.  The project will be supported by a research consultant and Social Determinants of Health Advisory Team.  Care Compass Network, a regional Medicaid reform organization serving the Southern Tier, is a primary partner.  The intent of the project is to position rural and small community organizations for an active role providing social determinant of health interventions through emerging value based payment networks. Qualifications: Bachelor’s Degree in public health, health planning, public administration, human services, etc. and three years related experience, or an acceptable combination of education and work experience. Experience in research and measurement is preferred. This position is eligible for all full time position benefits.
