Position: Community Health Worker 1

Job Posting: The Rural Health Network Is seeking a Community Health Worker 1 for Broome County to provide outreach to engage individuals enrolled in Medicaid  and uninsured county specific populations to efficiently access and health related services including, prescription medication, vision/dental services, financial assistance, health insurance, transportation, healthy foods, housing, and health advocacy. The Community Health Worker 1 will refer and facilitate participation in chronic disease case/self-management, care coordination and/or health education. The consumer relationship ranges from one time information and referral services, to short term onboarding of individuals with chronic physical and/or behavioral health conditions to access care coordination/case management. Associate’s Degree required, Bachelor’s Degree preferred in health, human services, or related field. This position is eligible for all full time position benefits.

Community Health Worker 2 will provide supervision to Community Health Worker – 1 staff and/or AmeriCorps Members. Provide support to Medicaid Members enrolling in an Opioid Use Disorder Cohort Management Project based in a hospital setting. Eliminate health care access barriers to primary care, treatment, and recovery services. Reduce stigma through embracing harm reduction model and low threshold approaches.

Minimum Qualifications: Bachelor’s Degree required in health, human services, or related field. Knowledge of Substance Use Disorders required. Valid Driver’s License, proof of automobile insurance, and use of reliable private vehicle for work-related travel purposes. Electronic medical record, Excel, interpersonal, organizational and communication skills required.
